What to expect at your appointment with a pharmacist:
- Review of every single medication you are on AND provide you with a printed copy of your medication list as well as digital access within the patient portal
- Review medication interactions and other pertinent information with you
- Counseling on lifestyle habits you can implement to help optimize your medications effectiveness
- Send your medication list to your primary care physician and any specialists you see for their records and review.

- Reconciled Medication List
- Identification of duplicate or expired medications
- Determine gaps in therapy recommendations
- Drug interactions findings and recommendations
- Adverse interaction and side effect findings
- Summary of counseling that we will go over with you
Who is this best for?
Someone who:
- is not familiar with their medication
